Ourera provides telemedicine addiction treatment in the Bay Area and across California.

Your relationship with substances may feel overwhelming right now, or it may be something you have worked hard to stabilize for years. Many people struggle with fear of relapse, fragmented care, long wait times, stigma, or the pressure of having to retell their story again and again across care settings.

We care for patients, age 16+, anywhere in the Bay Area with active or past addiction to opioids, alcohol, benzodiazepines, or stimulants.

How it works

  1. Schedule Your Visit

Schedule your initial visit, then complete your secure online enrollment forms that will be sent to you after scheduling.

Your enrollment and your form responses are confidential and treated as protected health information.

2. Meet Your Provider by Video

Complete your initial video consultation with your Ourera provider.

There are no pre-visit screenings or long intakes prior to meeting your provider. Meet your provider online for a full initial telemedicine assessment that will be billed to your insurance. You’re responsible for any co-pay or deductible obligation that your insurance plan requires.

3. Receive Your Personalized Treatment Plan and Prescription(s)

Receive your treatment plan, prescription(s), and orders for any recommended labs or referrals.

When indicated, we prescribe buprenorphine (Suboxone, Sublocade, Brixadi), naltrexone (Vivitrol), acamprosate (Campral), and long-acting benzodiazepine tapers (Valium, Librium) among other effective treatment options.

We work with partner programs to provide coordinated coaching, therapy, and contingency management when aligned with your goals.

Your provider will follow-up with you for additional visits on a schedule that’s catered to your needs and expectations.

Dr. Clear believes that addiction treatment works best when you stay in control.

His role is to offer clear information, thoughtful recommendations, and reliable support, never to tell you what your goals should be. Whether you’re seeking treatment that feels simple and straightforward or more structured and involved, he works with you to find an approach that fits your life, with honest discussion of the benefits and tradeoffs at every step.

A board-certified Family Physician and Addiction Medicine specialist, he founded Ourera to create personalized care plans that work for each patient, whether you’re managing withdrawal, tapering medications safely, or beginning recovery for the first or fifth time.

Before Ourera, Dr. Clear served as Chief Medical Officer of Bicycle Health, where he helped grow a nationwide telemedicine program caring for more than 15,000 patients. He brings that experience in scaling high-quality virtual care to a smaller, more personal setting, so you can get expert treatment from the comfort of home.

Outside of medicine, you are more likely to find him walking to his neighborhood farmer’s market, lingering over a slow morning coffee, or exploring great food spots around Oakland to enjoy with his wife and their chow-chow, Ray. Staying active and connected helps him show up as a grounded, approachable partner in his patients’ care.

FAQs

What conditions does Ourera treat?

We treat patients with current or a history of any substance use disorder (addiction).

Our most effective treatments address addiction to Opioids, or Alcohol. We also provide effective treatment for Benzodiazepine dependence and withdrawal.

For patients with addiction to Stimulants, the most effective current treatment is a structured therapy program that can be provided by a local in-person or telemedicine partner. Ourera offers you medical support for symptom control and overall health before, during, and after your behavioral therapy program, but Ourera does not provide behavioral therapy for stimulant use disorder directly.

Will telemedicine work for me?

Care for most patients with a substance use disorder (addiction) can be provided effectively through telemedicine. You just need consistent access to a smart device with internet access and video-call capability, whether it’s your phone, tablet, laptop, or desktop. If you’re unsure about telemedicine, we can talk about the benefits and risks in more detail at an initial visit and make an informed decision together.

How does it work?

Enroll Now to reserve your initial visit. You’ll receive a link to our secure enrollment forms to complete before your visit. All information you provide is considered protected health information and is kept confidential as per our Notice of Privacy Practices.

At the visit, you’ll meet your provider by secure video conference, discuss your care goals, align on a plan, then receive treatment recommendations and start care at the same visit. You’ll schedule follow-up and learn how to contact us for sooner care as needed. You’ll fill any prescribed medication at your chosen pharmacy.

Will Ourera be my primary care provider?

It’s preferred for most patients to also have a separate primary care provider (PCP) with an in-person office available. With your consent, Ourera will share records and coordinate with your primary care provider.

While Ourera will not be your official PCP, we can provide limited primary care services through telemedicine like treatment of mood disorders, high blood pressure, and some other chronic condition management when needed. Our service is not effective for emergencies or acute care.

Do you provide medication?

Yes, we prescribe medications that are safe and effective for your presenting condition(s). For substance addiction, depending on the substance, the most commonly prescribed medications may include buprenorphine (Suboxone, Sublocade, Brixadi), naltrexone (Vivitrol), long-acting benzodiazepine tapers (Valium, Librium), and acamprosate (Campral).

In addition to treatment of substance use disorders directly , we also may prescribe medications to treat other related conditions.

Do you take my insurance?

We are in-network with Aetna, Anthem Blue Cross, UnitedHealthcare (UHC) / Optum, and Medicare Part B throughout California.

We are working toward becoming in-network with Blue Shield of California and Cigna in early 2026.

We are working toward accepting Medi-Cal in Alameda County from mid to late 2026.

Is there a self-pay option?

There is. As long as you don’t have active Medi-Cal, any uninsured patient or any patient who does not wish to use insurance may choose to self-pay for services.

Is my care confidential?

Yes. From the time you submit a new patient request, your identity and all health information you provide us are considered protected health information and shared only as described in our Notice of Privacy Practices. We encourage coordinated care with your other providers, and we also respect that your medical record exists only for your benefit, and you’re in control.
